How to Sign Up for MyChart
You have a few different options to activate your own MyChart account.
Self-signup: Go to the signup page on the MyChart website and fill out the requested information. Once we match the information with our records, you will be on your way!
At your doctor's office: All Contra Costa Regional Medical Center & Health Centers patients receive a unique MyChart activation code in the "After Visit Summary" paperwork given to them following an office visit. Go to the MyChart website and follow the step-by-step sign-up instructions in your After Visit Summary.
Contact MyChart customer support: Call us toll-free (1-844-622-5465) or send us an email at MyChart.firstname.lastname@example.org saying you'd like to start a new MyChart account. After we confirm your identity, we will send you an email with an activation code. That's it!
